Quadriceps tendon rupture; Other names: Quadriceps tendon tear: An x-ray demonstrating quadriceps tendon rupture. Note the abnormal angle of the patella and soft-tissue swelling marked by the arrow.

Epidemiology incidence quadriceps tendon rupture is more common than patellar tendon rupture; demographics usually occurs in patients > 40 years of age; males > females (up to 8:1) occurs in nondominant limb more than twice as often; location of rupture 1994-03-01 Radiologic case study. The more common indirect mechanism results from forced flexion when the quadriceps is contracted. Rupture of the extensor mechanism of the knee must be diagnosed early in order to be successfully The noninvasiveness and accuracy of magnetic resonance (MR) imaging make it useful in detection of such ruptures. Normal quadriceps tendons have a laminated appearance on MR Skeletal Radiology, Vol. 43, No. 7. Fractures of the Patella. Case: A 51-year-old male construction worker with a history of chronic quadriceps tendon rupture presented with no active knee extension and tricompartmental knee osteoarthritis. He underwent simultaneous total knee arthroplasty and extensor mechanism allograft reconstruction.

Arthrography revealed an abnormal tract of contrast agent dissecting from the suprapatellar bursa into the soft tissue anterior to the patella and downward to a level 9 cm beneath the tibial plateau in a patient with complete rupture of the quadriceps tendon.

22.3). On ultrasound, patella alta results in the trochlear groove, normally hidden behind the patella, becomes visible, while examination of the tendon itself will demonstrate the discontinuity ( Fig. 22.4 ). Ultrasound has been shown to be sensitive in detecting quadriceps tendon rupture. It shows both complete and partial ruptures, soft tissue hematomas, tendinopathies, soft tissue tumors, myositis ossificans, and fascial defects. Incomplete intrasubstance tears of the rectus femoris and complete tears of the rectus femoris and quadriceps tendon are visible as increased signal intensity on the T2 images. The laminated configuration of the quadriceps tendon allows distinction between partial and complete tears; both may be accompanied by edema and hemorrhage, which manifest as increased signal intensity on T2-weighted images. Ruptures of the patellar tendon also manifest as increased signal intensity on T2-weighted images.

Note the abnormal angle of the patella and soft-tissue swelling marked by the arrow.

Following rupture, a high-riding patella may be detected on clinical examination or conventional radiography as a result of retraction by the quadriceps mechanism (Fig. 22.3). Vastus medialis obliquus (VMO) tendinous tear-MRI Thursday, March 08, 2012 Musculoskeletal MRI, quadriceps injury, VMO tear Rupture of the quadriceps tendon was first reported in the English literature by Samuel in 1838. The quadriceps tendon usually ruptures transversely at the osteotendinous junction.
